Form 1076 Filing Instructions

The 2010 Annual Municipal Premium & Tax Report (Form 1076), which allocates Louisiana premiums written to the various municipalities and parishes, can be filed online.

In August, 2008 the Department redesigned the online filing system that now uses your e-mail address as your login. The old passwords and login IDs that were issued previously are no longer valid. If you have not signed up since then to the new Industry Access portal, you will need to do so in order to file the Form 1076 and the 1061 online. To get started, click on "Industry Access" from our homepage. Then click on "Sign Up" at the top and fill in the information requested, including your e-mail address and a password that you choose. After entering all of the information, click on "Sign Up" at the bottom of the entry. This will send an e-mail to the Department. Shortly, you will receive an e-mail reply verifying that you can access the system. Then go to the Industry Access link again with your e-mail address and the password that you have chosen. From here, you can download instructions on how to proceed to file the tax forms.
