Requesting Public Records From The Louisiana Department Of Insurance

General: The Louisiana Department of Insurance (LDI) processes public records
requests during regular business hours each business day. Public records requests
are not processed on Saturdays, Sundays, or legal public holidays. Requests must
be made in writing and can be made online or by using the LDI form for Public Records Requests.
Responses are made to the person making the request. The Louisiana Public Records
Law is at La. R.S. 44:1
et seq.

Response to Request: To respond to a request for access to the LDI public
records, the LDI needs certain information from the requestor. LDI staff must search,
retrieve and view records. The requestor will be informed of the status of the request within
three days of receiving it. The requestor is notified of applicable fees due. See
"Fees" below. Upon receipt of applicable fees, public records are available to the
requestor. If payment is not received within 10 working days after notice is forwarded
by the LDI to the requestor, it may be necessary for the requestor to initiate a new request.
Estimate of Fees: If an estimate of costs is requested, the requestor must
specify that and copies will not be made until after the requestor provides written
confirmation of the order.
Online Requests: Online requests may be made from this website, www.ldi.state.la.us. Continue below to accept terms and
make an online request.
Requests by U.S. Mail, courier, facsimile, or in person: Requests may be made by U.S. Mail,
courier or in person to Custodian of Records at the above address. Requests by facsimile may be made to
225.342.1632. For requestors’ convenience, the LDI forms for public records requests are: (1)
Public Records Request Form
and (2) Certification for Free or Reduced Rates.
Requests are NOT made to individual LDI staff E-mail addresses because individual members of the
LDI staff may be unavailable when an E-mail is received, resulting in the potential for the public
records request response team being unaware that the request has been made.
Fees:
Uniform Fee Schedule; Administrative Code: The Uniform Fee Schedule for copies
of public records is published in the Louisiana Administrative Code at LAC 4:I.301. After clicking

Local Access to LDI Public Records (View the Public Record at LDI): There
is no fee assessed to view the public record at the LDI office.
Free or Reduced Rate: In order for the LDI to consider a free or reduced
rate copy of the public record, the requestor must (1) qualify and (2) certify qualification
for free or reduced rate. See LDI
Certification for Free or Reduced Rates form. The requestor must also complete
the Public Records Request Form.
Upon receipt of a request and review of records requested, the LDI will forward
an invoice to the requestor. Fees are then paid by check or money order payable
to the Louisiana Department of Insurance and forwarded to:

Payments cannot be made online at this time.
