Louisiana Citizens Property Insurance Corporation Assessment Forms
Filling out an INS form takes only a few minutes and asks for basic information. Once you have filled out the form, mail it to the Louisiana Department of Revenue (the address is on the form). Be sure to attach a copy of your insurance declaration page showing the assessment has been included in the insurance premium. Your insurance agent can provide you with declaration pages for prior years if you don't have them.
